How do I sign up for CareSpace (the Mission patient portal)?

Patients wishing to register for the patient portal, may ask any employee for assistance while in the office. You may call our office at 515-282-2921 and ask to speak with the clinical analyst in charge of the portal during office hours (8AM-5PM) also.  You will need to provide your email address in order to complete the registration.
