How do I sign up for MyChart (patient portal)?

MyChart is a free, easy way to manage your health care form the comfort of home, 24/7. Go to uihc.org/mychart and click on the “Sign up” to get started. CareSpace is the former medical record platform for Mission Cancer + Blood. If you were a patient prior to June 2, 2025, some of your information may still be found in Care Space. To sign up for Care Space, you may call our office at 515-282-2921 and ask to speak with the clinical analyst in charge of the portal during office hours (8AM-5PM) also. You will need to provide your email address in order to complete the registration.
